First Hint:
You changed/installed something and it doesn't work?
Did you run texhash (same as mktexlsr)?
If not, do it – it rebuilds TeX's lsR database, used by kpathsea to find your files. (In MikTeX there's a similar command, "rebuild database" or the like.)

Open bugs (for 2005.06.08):
- enco-cyr.tex: x2 has a resetcaserange for 0-127 as well as the upper 8-bit
- core-des.tex: there is a missing \noexpand in \special@@descriptionhandler
- core-lst.tex: a bug is causing numbers to appear in front of \Topics in s-pre-05
- fonts/map/../context/pdftex.map should be deleted from the distribution
